Braemoor Admits patients with a physician= s order and proper consent, twenty-four (24) hours a day, 7 days a week from: hospitals, emergency rooms, physician offices, and home.

Reasons for admissions include:

                Short Term Rehabilitative Care

                Complex Medical Care

                Respite Care

                Traditional Long Term Care

                Hospice Care                

 

SHORT TERM REHABILITATION CARE

The Average Length of stay for a short term rehabilitation patient is 10 - 14 days (depending on his/her diagnosis), then discharge home.

Braemoor specializes in orthopedic care providing services for short term rehabilitation for total hip and total knee replacements, as well as other short term rehabilitation issues.

If you are thinking of having general or orthopedic surgery, and would like to know what your recovery process may include, Braemoor will meet with you prior to hospitalization to review your recovery plan, tour you through our inpatient program, introduce you to the nursing and rehabilitation team and answer questions on your insurance coverage.

If you need services after your inpatient stay has completed, Braemoor is licensed to provide Outpatient Services in our on-site 7000 sq ft rehabilitation clinic. This helps to maintain the continuity of your care plan with the same rehabilitation team your had during your inpatient stay.

COMPLEX MEDICAL CARE

Sometimes an individual is admitted for care following extensive surgery. The patient= s needs may be complicated and it is not appropriate to provide the care in the home setting, but the patient does not require long term care. Braemoor can provide complex medical care on it= s sub-acute nursing unit. The length of stay may be greater than that required for short term rehabilitation, but the care plan goal is for home discharge.

RESPITE CARE

Braemoor provides temporary services for short term respite patients. Many times patients do not require the continuous services of a skilled care facility and are cared for at home by family members. From time to time the family members have commitments that render them unable to provide the care for a few weeks at a time. Braemoor can help to manage the patient= s care needs while the family attends to it= s other commitments.

TRADITIONAL LONG TERM CARE

Some health care can be too technical or too difficult to provide in the home setting, and Assisted Living may not meet the health care need. While families try to keep other family members at home as long as possible, twenty-four hour care can be trying. Braemoor provides traditional long term care to seniors with a care plan that supervisors the resident= s medical, nursing, restorative, dietary, and activity needs.

HOSPICE CARE

Hospice Care can be provided in the home or the skilled care facility depending on the resources available to the patient and the family. Braemoor has contracts with different Hospice Organizations to provide care in the skilled nursing setting. A resident can be admitted specifically for Hospice Care, or can transition into Hospice Care if necessary.
