Dear Family Member / Responsible Party:

 The Massachusetts Department of Public Health has contracted with an organization in South Portland, Maine called Market Decisions LLC, to conduct a survey of family members of nursing home residents, to determine their satisfaction with the quality of care that their loved ones receive.  This survey will be conducted throughout the State of Massachusetts; participation in this survey is voluntary on the part of the nursing home.  Results of the survey will eventually be posted on the Internet comparing the facilities.  Braemoor has elected to participate.

 The Department of Public Health considers this survey an oversight activity, and has asked nursing homes throughout the State to provide the names, addresses, and telephone numbers of all long term care resident’s responsible parties in each facility.  The DPH has stated this information can be provided without violating privacy laws.  You may expect a call from a representative of Market Decisions sometime over the next few weeks to ask your opinion of the care your family member is receiving at Braemoor. 

 The Department of Public Health feels this survey will assist consumers in researching facilities when looking for placement for family members.
