Pearl Street Brockton, MA 02301 Phone: 508-586-3696 Fax: 508-584-0470 IMPORTANT NOTICE MEDICARE PART D PRESCRIPTION DRUG PROGRAM (PDP) The Medicare Modernization Act signed into law the New Medicare Part D Prescription Drug Program on 12/8/03. This program will go into effect on JANUARY 1, 2006. During October you will receive important correspondence from Medicare (CMS). Please do not discard this information, in some cases you may have a responsibility to react. Medicare believes this is a cost effective drug program for eligible individuals. The Prescription Drug Program (PDP) creates several drug plans with minimal premiums. A beneficiary may review each PDP and determine which program best fits his/her needs based on the types of medications the individual is currently taking. In many cases enrollment is voluntary. You are eligible to enroll in Medicare Part D if: you are entitled to Medicare Part A or enrolled in Part B, or if you are enrolled in a Medicare Advantage Plan (Secure Horizons, Blue Care 65, etc) without Prescription drug coverage All MassHealth (Medicaid) beneficiaries are eligible for this program. If you only have MassHealth with no other insurance, you will not notice any change. If you are a dually eligible beneficiary, i.e. you have both MassHealth and Medicare (which applies to many residents) you will be automatically and randomly enrolled by CMS, into a PDP; the program is not voluntary. It is important to note that the PDP you will be enrolled into may or may not meet your medication needs, and you may be required to react to change your PDP. This will require communication with Braemoor and your physician. This is an important Program. Braemoor will hold a Family Information Session on Wednesday November 2 at 4:00pm and a second session at 7:00pm to review the new Prescription Drug Program. All families, residents, and staff are invited. There are several ways to communicate with Medicare (CMS) to obtain additional information including; www.medicare.gov, www.cms.hhs.gov/medlearn/drugcoverage.asp, and 1-800 MEDICARE (800-633-4227).
