NURSING SERVICES Admissions Services Physicians
B raemoor's reputation for Nursing Services has been recognized in the Community since 1939.Over the years, our Nursing 's A Commitment to Quality has kept pace with the industry from the days of traditional bedside care, to current day short term, complex and sub-acute care.
ADMISSIONS... Referrals for admissions are received from hospitals, emergency rooms, physician offices, third party payers, and family members. All referrals are screened for medical/rehabilitative necessity and appropriateness of care. Once screened, patients may be admitted 24 hours a day, seven days a week with a physicians order.
SERVICES... The Nursing Department is qualified to provide a wide range of services including: * Pain Management * Intravenous Therapy, * Total Parental Nutrition, * Blood Glucose Monitoring, * G-Tube Re-insertion * Bowel and Bladder retraining * Oxygen Therapy * Medication Monitoring * Post Cardiac Monitoring * Orthopedic Care * Post Surgical Care * Wound Care * Hospice Care * Pulmonary Care Braemoor 's Restorative Nursing programs assist the patient in the transition from intensive rehabilitation therapy to programs that help the patient continue with the goal of independence while preparing for home discharge.
PHYSICIANS... Braemoor 's Nursing Staff works directly with the patient's Primary Care Physician to ensure the most up to date resources are available in support of the patient's care plan.
Nursing Services are supervised by Braemoor 's Medical Director.
Skilled patients are assessed at team meeting, where nursing managers identify and supervise the patient 's treatment plan and coordinate care with other disciplines including physical, occupational and speech therapies, social services, and dietary. The goal is to ensure the care plan stays current and progresses with the patient.
DISCHARGE... For those patients with a plan for home discharge, Nursing offers patient and family teaching, including diabetic and colostomy instruction, to ensure that once home, the patient can perform his or her functions as independently as possible, and that the care giver is comfortable assisting the patient.
